Abstract
This publication examines the problems and issues associated with the transitions experienced by adolescents and young adults with chronic and disabling diseases (paediatric cancers, rare diseases, etc.). The eight chapters of the book are divided as follows: medicalhospital transitions, family transitions, school/university transitions. The multifactorial aspect of the concepts of transitions and support needs to be considered from a variety of perspectives, so several disciplines are brought together in this volume by researchers and field workers: education and training sciences, psychology, medicine and sociology.
